An analysis using genetic evidence reported that GLP-1 receptor agonists achieve weight loss primarily by reducing fat mass rather than lean mass. Separately, a large real-world study reported that semaglutide was associated with a lower risk of bone fractures in people with type 2 diabetes.
What this actually means
This goes directly against one of the most widely repeated claims about these drugs: that they strip muscle along with fat and leave you weaker.
That claim was never baseless. Any substantial weight loss involves losing some lean tissue, and that is worth managing with protein intake and resistance training. But the version circulating online, that GLP-1s specifically waste muscle, is not what the evidence shows. The loss is disproportionately fat.
The fracture finding points the same way. If these drugs were meaningfully weakening people, you would expect more broken bones, not fewer.
Worth noting who benefits from the muscle-wasting story. It is a very effective way to sell peptides, protein products and training programmes to people already anxious about a medication they are taking.
