The short answer
Same drug, and the tablet is not simply the easier option. It removes the needle and adds a daily constraint most coverage skips: it has to be swallowed whole on an empty stomach after fasting for at least eight hours, with only a sip of water.
If a needle is what has been stopping you, the tablet is a genuine change. If your mornings are chaotic, it may be harder to live with than one injection a week.

02What is the same
- The drug. Semaglutide in both.
- The licence. Both licensed in the UK for weight management, with the same BMI-based eligibility: 30 or above, or 27 to 30 with at least one weight-related condition.
- The mechanism and the side effects, since it is the same molecule.
- The regain problem. Both stop working when you stop.
- Private only for the tablet, and heavily restricted on the NHS for the injection. Neither is easy to get funded.

The reason for the fasting rule is chemistry. Peptides are destroyed by digestion, so an oral semaglutide tablet relies on an absorption enhancer and needs an empty stomach to work at all. Food in your stomach does not reduce the effect slightly, it undermines the delivery.
Orforglipron, approved in the US in April 2026 and under MHRA review, is a non-peptide taken as a tablet with no food restrictions, because it is not a peptide and survives digestion on its own. If the fasting window is the dealbreaker, that is the thing to watch for rather than this.
04What the evidence says
Oral semaglutide for weight management was assessed by the MHRA and licensed on 11 June 2026. The MHRA said people already on the weekly injection privately can move straight to the highest tablet strength.
Separately, SOUL reported cardiovascular outcomes for oral semaglutide, and EVOKE tested whether it slowed Alzheimer's progression and found no effect. The negative Alzheimer's result is worth recording because it is the kind of finding that gets quietly dropped from coverage of this class.
05Which suits whom
- Needle phobia is a real clinical reason, not squeamishness, and it stops people accessing treatment. The tablet exists partly for this.
- A settled morning routine helps enormously. The eight-hour fast plus waiting before food is a daily commitment.
- Weekly beats daily for a lot of people. One thing to remember on a Sunday is easier than a rule every morning.
- Funding. The tablet is private only. If NHS funding is your route, the injection is the only one in play.
06Getting either in the UK
The tablet went on sale in the UK on 6 July 2026 and is available privately through registered pharmacies, not on the NHS. The injection can be NHS-funded under NICE criteria and local commissioning, and is also available privately.
The National Pharmacy Association surveyed 310 pharmacy owners on launch day, and 97 per cent said they were worried the tablet would drive an increase in fake and unlicensed weight-loss pills on the black market. A tablet is far easier to counterfeit convincingly, and far easier to post, than an injection pen.
07The honest bottom line
Same drug, same licence, same eligibility. The tablet trades a weekly needle for a daily fast, and gives up the possibility of NHS funding.
If needles are the barrier, that trade is obviously worth it. If they are not, the injection is simpler to live with and is the only version anyone might pay for on your behalf.
