The American College of Physicians published a living clinical guideline establishing semaglutide and tirzepatide as conditional first-line pharmacological options for obesity. The guideline sits alongside separate specialty frameworks published the same month setting out how obesity care should be integrated across services.
What this actually means
This is American guidance and does not bind anyone in the UK, where NICE decides.
It still matters, for one reason: it marks the point where these drugs stopped being treated as a last resort. First-line means considered early rather than after everything else has failed. UK guidance has moved in the same direction, with tirzepatide prescribing for obesity entering the GP contract in April 2026.
Note the word conditional, which is doing real work. It signals the guideline authors judged the evidence supportive but not definitive, and it is the honest position given how recent all of this is. A guideline saying conditional is more trustworthy than one that overclaims.
